Summary of Benefits

Southwestern Behavioral Healthcare, Inc. offers an attractive benefit program to full-time employees working an 80% schedule or 32 hours per week. The following is a summary of the benefits currently offered.

Health Insurance

Must select from a choice of networks.
Major Medical Benefits:

     $500 Family limit

Cost Sharing After Deductible:
Plan pays 80% of first $2,500 per person; $5,000 family limit (plan pays 60% for out of network providers).  Plan then pays 100% for additional expenses incurred during the same calendar year.

Basic Benefits: Brand prescription drugs are covered at 60%; generic drugs covered at 80%;  Annual Wellness Benefits 80% of the first $300 then 50% of the next $700 employee and covered spouse.

Dental Insurance

 

Two dental insurance plans are offered to cover employees and dependents for a variety of dental services.  Plan 2 is the basic plan with Plan 6 offering premium services.

First of the month following three months of employment

Insurance

A vision insurance plan is offered to cover employees and dependents for a variety of vision services.

Life Insurance and
AD&D

Equal to one times annual salary (rounded to next higher $1,000) to a maximum of $50,000.
Provides an additional benefit if death or dismemberment occurs as a result of an injury within 365 days.

First working day following 3 months of employment

Voluntary Life Insurance and AD&D Employee, Spouse and Children

Life insurance is available in $10,000 units from a minimum of $20,000 to a maximum of $500,000 with a guaranteed issue of $100,000. If you elect coverage for yourself, you can buy up to 50% of that amount for your spouse in $5,000 units to a maximum of $250,000. If you elect child coverage, your children are eligible for coverage of $1,000, $5,000, or $10,000 each. Proof of coverage is required for amounts greater than $100,000.

Long Term Disability

After 3 consecutive months of total disability, plan pays 60% of basic earnings to a maximum monthly benefit of $6,000.

First working day following 3 months of employment

Section 125 - Flexible Benefit Plan

The Flexible Benefit Plan allows certain qualified expenses to be paid with pre-tax dollars. The pre-tax dollars are not subject to taxes, which result in an increase in spendable income. Health, Dental and Vision Insurance Premium Plans - Employees who carry Southwetern health and/or dental insurance may have payroll deduction for premiums on a pre-tax basis. Flexible Spending Accounts - Employees may enroll in Health Care Account and Dependent Care Account.

Employees are immediately eligible for health and dental insurance premium options. Expense account options are available during annual open enrollment period.

401K Plan

Southwestern offers a 401K Plan with two opportunities for contributions by Southwestern, an employer contribution and a match contribution.

Southwestern will make a 3% employer contribution to the plan each pay based on eligible compensation.

Southwestern will make an annual match contribution of 50% of the employee's Elective Deferral. The maximum match will not exceed 50% of 5% of their compensation.

Employees will be enrolled on January 1st or July 1st following completion of 12 months of continuous employment and 1,000 hours of service.

401K Plan Employee Deferral

All employees may contribute to a tax deferred 401K or Roth 401K through payroll deduction. Minimum deferral is $12.50 per pay.

First of the month following employment.

Holidays

The following holidays are observed by Southwestern:
New Year's Day
Memorial Day
Independence Day
Labor Day
Thanksgiving Day and the following  Friday
Christmas Day

Immediately as long as in pay status the scheduled day before and after holiday. Hourly employees are paid 2 times their rate of pay for working the holiday.

Paid Time Off (PTO)

Matrix Levels 1-16 & A-F will earn per pay period:

Matrix Levels 17-32 will earn per pay period:

All employees will earn at a rate of 3.00 hours per pay period to a maximum of 200 hours

Employees are eligible to take PTO time after 6 months of employment.

Extended Illness Bank All employees will earn at a rate of 3.00 hours per pay period to a maximum of 200 hours. Upon employment

Employee Assistance Program

The program offers employees and members of their families assessment, short-term counseling, and referrals for personal problems
