Who to Contact
Our Patient Financial Services Department is responsible for medical billing and collections at Southwestern. We are here to help you with any questions you may have regarding your statement, insurance or other financial assistance needs. Please contact us directly at 812.436.4293 for assistance.
How to Apply
On your first visit you will be asked to provide Southwestern with your proof of income. You can provide this information to us in different ways including:
- A current paycheck stub or coy of your W-2
- A WorkOne release (if you are currently unemployed)
- A copy of your SSI benefit statement
Southwestern offers a sliding fee discount based on household income to those who do not have health insurance or other third party coverage and to those who have insurance which does not cover behavioral healthcare or specific behavioral healthcare services. Proof of income must be provided to receive the discount which is based on 133% Federal Poverty Guidelines with a minimum fee of $20 per service.
Southwestern offers additional fee discounts through an Application for Additional Fee Reduction. You can ask your therapist, a support staff member or a Patient Financial Services representative for an application form.
Once you turn in the application, it will then be reviewed and a Patient Financial Services representative will contact you to discuss whether or not you are eligible for an additional fee reduction.
