Who to Contact

It is Southwestern Behavioral Healthcare’s policy to serve all patients, regardless of ability to pay. We have many resources to assist with the affordability of services. 

 

Our Patient Financial Services Department is here to assist you Monday through Friday with any questions you may have regarding your statement, insurance or other financial assistance needs. Please contact us directly at (812) 436-4293 for assistance.

Insurance Billing

Many insurance policies will pay for all or portion of our fees.  It is our goal to make sure that every patient understands their treatment coverage and out of pocket expenses. We are here to help you feel more comfortable with this process.

 

Southwestern Behavioral Healthcare contracts with most commercial insurance providers and accepts Medicare, traditional Medicaid, Healthy Indiana Plan (HIP), Hoosier Healthwise, and Hoosier Care Connect.  The “regular eligibility determination” process for Medicaid coverage will begin April 1, 2023. FINANCIAL ASSISTANCE

Private Pay Rates:  Southwestern Behavioral Healthcare offers Private Pay rates which are discounted fees available to those without insurance or with an insurance plan that does not cover certain behavioral health services.

Sliding Fee Discounts:  Southwestern Behavioral Healthcare also offers sliding fee discounts for services based on household income and size up to 200% Federal Poverty Guidelines, with established nominal fees. 

Private Pay rates and Sliding Fee Discounts are offered to all patients unless prohibited by an insurance plan that designates a co-pay, coinsurance or deductible responsibility.

Determining Household and Annual Income and Size: 

If the individual receiving services is 18 years of age or older or legally emancipated:

 

  • Household size includes the person receiving services, their spouse and dependents/guardianships age 17 and under living at the residence.

  • Household income includes the person receiving services and their spouse living at the residence.

 

If the individual receiving services is 17 years of age or under:

  • Household size includes the person receiving services, their parents/legal guardians and any other dependents/guardianships of the parents/legal guardians 17 or under living at the residence.

  • Household income includes the income of the parents/legal guardians living at the residence.

Proof of income is required to verify eligibility for the sliding fee discount.   You can provide this information to us in different ways including:

  • Current paycheck stubs or copy of W-2

  • A WorkOne release (if you or a spouse are currently unemployed)

  • SSI benefit statement

Additional Fee Reduction

Southwestern Behavioral Healthcare offers additional fee discounts for financial hardships through an Application for Additional Fee Reduction. 30 days of current bank statements are required to verify eligibility. You can ask your therapist, a support staff member or a Patient Financial Services representative for an application form. Once you turn in the application and required financial documents, a Patient Financial Services representative will contact you to discuss whether or not you are eligible for an additional fee reduction.

Good Faith Estimate of Charges

According to Indiana law, we must provide uninsured and private pay clients a Good Faith Estimate of charges for non-emergency care within 5 business days upon request.  This Good Faith Estimate is based on information known at the time the estimate was created, is not binding and determined based on the Private Pay rates and any estimated financial assistance shown above. To request a Good Faith Estimate of charges for any scheduled services, please contact our Patient Financial Services Department at (812) 436-4293. Please be prepared to tell Patient Financial Services representative the the client name, date of birth, location of the appointment and what services are scheduled so that they are best able to provide this estimate to you.

Payments and Payment Plans

After your fee is established, you will be asked to pay your portion of charges at the time of each service.  If you have any questions about your fees or this process, our Patient Financial Services department is available Monday through Friday at (812) 436-4293.  Payment plans are available upon request.
