INTEGRATED CARE ENTITY GRANT GIBSON
Through the Primary Care Behavioral Health Initiative, we are able to improve the mental health and well‐being of our clients by increasing collaboration among primary health care and behavioral health care providers, and consumers. Evidence gathered over the last 10 years shows that the integration of primary and behavioral health care can improve access to primary care (especially for behavioral health patients), better coordination of care and doing so could potentially the reduction of health care costs. We currently offer these services for all our clients at our Gibson County office and hope to make it available to clients at all our facilities within the next five years.
